In recent months there have been 2 other publications of note:

 

Quantitation of serum free light chains in combination with protein           electrophoresis and clinical information for diagnosing multiple myeloma in a general hospital population.

Piehler, A. P., et al. Clin Chem, 2008. 54(11): p. 1823-30.

 

Piehler et al. evaluate an approach for laboratory diagnosis of monoclonal            gammopathies based on serum protein electrophoresis, serum FLC tests and other clinical and biochemical information. Serum FLCs were only requested when SPE or clinical or other biochemical findings suggested monoclonal gammopathy. This     strategy enabled a 75% reduction in the number of serum FLC tests required but still allowed efficient diagnosis of monoclonal gammopathies. Click here to order your copy (Code MKG486).

 

‘Light chain escape-multiple myeloma’- an escape phenomenon from plateau phase: report of the largest patient series using LC-monitoring.

This report summarises the largest case series of 10 multiple myeloma patients who developed light chain escape at disease relapse. The authors conclude that the early detection of light chain escape through serial serum FLC measurements may prevent the occurrence of unnecessary complications, such as renal impairment or           symptomatic bone lesions. The key recommendations are:-

 

1. ‘The serum FLC assay in combination with serum PEL and serum IFE is sufficient to screen for pathological monoclonal plasmaproliferative disorders other than AL which requires all the serum tests as well as 24 h urine IFE.’ 

2. ‘The serum FLC assay should be measured at diagnosis for all patients with MGUS, smoldering or active multiple myeloma. Solitary plasmacytoma and AL amyloidosis.’

3. ‘Serial FLC ascertainment should be routinely performed in patients with AL      amyloidosis and multiple myeloma patients with oligosecretory disease.’

4. ‘It should also be done in all patients who have achieved a CR to determine whether they have attained a stringent CR.’

Quantitative assessment of serum and urinary polyclonal free light chains in patients with chronic kidney disease.

This study describes for the first time polyclonal serum FLC (free light chains) in     patients with chronic kidney disease. Serum concentrations of both kappa and lambda FLCs increased with worsening renal function. Due to the loss of preferential      glomerular filtration of kappa FLC monomers over lambda dimers with renal           impairment, the median FLC ratio increased with decreasing renal function. Serum free light chain measurement aids the diagnosis of myeloma in        patients with severe renal failure.

This study examines the diagnostic utility of serum FLC in the management of patients with dialysis-dependent acute renal failure. Use of a modified range for the FLC κ/λ ratio (0.37 – 3.1) increased the diagnostic specificity of the serum FLC assay with no loss of diagnostic sensitivity, allowing rapid identification of Multiple Myeloma as  the underlying cause of acute renal failure.
