MGUS A monoclonal gammopathy precedes multiple myeloma in most patients. Weiss BM, Abadie J, Verma P, Howard RS, Kuehl WM. Blood 2009;113:5418-22 Monoclonal gammopathy of undetermined significance (MGUS) precedes Multiple Myeloma: a prospective study. Landgren O, Kyle RA, Pfeiffer RM, Katzmann JA, Caporaso NE, Hayes RB, et al. Blood 2009;113:5412-7
In the same edition of Blood, Weiss and Landgren both report analyses examining the evolution of MGUS into multiple myeloma (MM)… more
Renal Treatment of acute renal failure secondary to multiple myeloma with chemotherapy and extended high cut-off hemodialysis. Hutchison CA, Bradwell AR, Cook M, Basnayake K, Basu S, Harding S, et al. Clin J Am Soc Nephrol 2009;4:745-54 - Order: MKG516
This prospective pilot study assessed the combination of standard chemotherapy with extended haemodialysis using a high cut-off dialyser (HCO-HD) on serum FLC concentrations and renal recovery in 19 patients with biopsy-proven cast nephropathy and dialysis-dependent acute renal failure… more
Are renal reference intervals required when screening for plasma cell disorders with serum free light chains and serum protein electrophoresis? Abadie, J.M., K.H. van Hoeven, and J.M. Wells, Am J Clin Pathol, 2009. 131(2): p. 166-71.
A modified renal reference range for the FLC κ/λ ratio (0.37-3.1) was proposed for patients with renal impairment (Hutchison et al. 2008). Abadie et al. evaluated this range when screening for plasma cell disorders… more
Screening Utility of serum free light chain analysis when screening for lymphoproliferative disorders. Robson, E.J.D., et al., Lab Med, 2009. 40(6): p. 325-329.
Robson et al. examined the utility of screening for lymphoproliferative disorders using a combination of serum protein electrophoresis (SPE) and serum FLC tests. Of 653 serum samples, serum FLC analyses significantly contributed to diagnosis in seven patients… more
Inaccuracies in 24-hour urine testing for monoclonal gammopathies. Siegel, D.S., et al., Lab Med, 2009. 40(6): p. 341-344.

Amyloidosis Identification of amyloidogenic light chains requires the combination of serum-free light chain assay with immunofixation of serum and urine. Palladini, G., et al., Clin Chem, 2009. 55(3): p. 499-504.
The authors analysed the diagnostic sensitivity of serum/urine IFE and the serum FLC assay in 121 AL amyloidosis patients… more |

World Congress of Nephrology 22-26 May - Milan, Italy The World Congress of Nephrology (WCN) was jointly organized by ERA-EDTA and ISN and took place 22–26 May 2009 in Milan, Italy. This meeting was the second WCN to be held in Europe and had a high level of scientific content covering an expanding list of topics related to all major areas of basic, translational and clinical sciences relevant to nephrology and several fundamental aspects of biological sciences. Each day incorporated a plenary lecture given by internationally recognised scientists. Approximately 12,000 delegates attended which generated a great atmosphere.
Binding Site took the opportunity to work alongside Gambro in promoting both Freelite and the Theralite haemodialysis membrane, and also the EuLITE trial. Gambro also hosted satellite symposia where both Dr Colin Hutchison and Dr Paul Cockwell discussed their work and the patient benefits seen when using a combination of velcade-based chemotherapy, Theralite and Freelite.
There were many visitors to Binding Site’s exhibition stand, so thank you to those of you who came and discussed how Freelite can be used in support of nephrology patients. Click here to view a document discussing Freelite, Theralite and EuLITE. |

European Haematology Association (EHA) 14th Congress, 4-7 June 2009 - Berlin, Germany The annual EHA congresses have steadily grown in size and quality and approximately 7000 attended this year. The Scientific Program Committee and its Advisory Board compiled an exciting program covering the broad spectrum of benign and malignant haematology incorporating Educational and Plenary sessions, Meet-the-Expert sessions and Lunch Debates. New data was provided in the Hematology-in-Focus, and Clinical Trial Updates and the Molecular Hemopoiesis workshop also offered a specific opportunity for the presentation of cutting edge research results.
A large number of abstracts (over 2000) submitted for this EHA Congress testifies to the importance of these meetings, not only for European haematologists but for haematologists world-wide. The Scientific Program Committee also acknowledged the increasing quality of the data and work that was been submitted.
EHA also offers webcasts, and vodcasts (video and audio) of the Education Sessions, José Carreras Lecture, Presidential Symposium, EHA-ASH Joint Symposium, and the Plenary Sessions. Webcasts are open to all EHA members and to participants of the 14th Congress in Berlin. If you attended then to access the webcasts and vodcasts you need your invitation code, which can be found on the back of your congress badge if you still have it or check the website for more information.
Binding Site was pleased to have a significant stand presence which attracted a lot of visitors and we thank you for joining us to pick up the new International Myeloma Working Group guidelines, new papers on Freelite and to be introduced to Hevylite.
Next year’s meeting, the 15th Congress of EHA, will be held 10-13 June 2010 in Barcelona, Spain and the Congress President will be J San Miguel. |

Euromedlab, 7-11 June 2009 - Innsbruck, Austria The scientific program of EuroMedLab 2009, the 18th European Congress of Clinical Chemistry and Laboratory Medicine, was extremely varied with 30 different sessions over 4 days plus a daily plenary lecture and nearly 900 posters. The diagnostic industry presented their latest products and also hosted 27 individual workshops.
Binding Site sponsored a popular session entitled 'Recent advances in the management of Free Light Chain Disease’ which was attended by over 130 delegates. Our eminent speakers were: Prof. Drach, Medizinische Universität Wien, Austria (Application and clinical significance of serum free light chain testing), Dr. Jerry Katzmann, Mayo Clinic, US (The new international Myeloma Working Group Myeloma Guidelines and how they impact the laboratory) and Dr. Paul Cockwell, University Hospital, Birmingham, UK (Treatment of acute renal failure secondary to multiple myeloma with chemotherapy and extended high cut-off haemodialysis).
The next EuroMedLab meeting will be held in Berlin, 15-20 May 2011. |

Guidelines: If you don’t already have a copy of the new guidelines please click here to request your copy of MKG492
The key recommendations are:-
1. ‘The serum FLC assay in combination with serum PEL and serum IFE is sufficient to screen for pathological monoclonal plasmaproliferative disorders other than AL which requires all the serum tests as well as 24 h urine IFE.’ 2. ‘The serum FLC assay should be measured at diagnosis for all patients with MGUS, smoldering or active multiple myeloma, solitary plasmacytoma and AL amyloidosis.’ 3. ‘Serial FLC ascertainment should be routinely performed in patients with AL amyloidosis and multiple myeloma patients with oligosecretory disease.’ 4. ‘It should also be done in all patients who have achieved a CR to determine whether they have attained a stringent CR.’ |
